In this role you will support community members who may face barriers accessing health services or participating in activities that promote wellbeing. You'll play a vital part in helping clients strengthen their social and emotional wellbeing by linking them to existing services, supports, and community programs that enhance health outcomes. Working as part of a multidisciplinary team you will manage a small caseload using an assertive outreach model, engage in community awareness initiatives, and advocate for clients in a culturally respectful way. You'll also have the opportunity to build capacity within communities by mentoring Yarnangu employees, supporting colleagues, and working collaboratively with other service providers to develop community-led solutions. About You: Tertiary qualification in Social Work and eligible for membership of the Australian Association of Social Workers. Experience in delivering therapeutic interventions and a holistic, client-centred model of care. Strong analytical and assessment skills, with the ability to quickly establish effective therapeutic relationships with clients, families, and colleagues. Knowledge and understanding of Aboriginal cultures, Aboriginal Health, and primary health care, with the ability to work effectively across multidisciplinary teams. Ability and willingness to travel regularly by road and light aircraft to remote locations. Current manual driver's licence and ability to drive a 4WD on long unsealed roads.
